RN Concierge Nursing Services
Gracepoint Care Group provides personalized RN-led concierge care designed to help clients stay safe, organized, and supported at home. Services are tailored to each individual and may include:
- Medication box setup and refill with RN review
- Wellness check-ins and vital sign monitoring
- Health education and support for new diagnoses
- Coordination of providers, home health, and community resources
- Functional and safety assessments in the home
- Appointment preparation, organization of questions, and follow-up review
Our concierge model focuses on guidance, advocacy, and peace of mind. We work alongside families, physicians, and care teams to help clients navigate the healthcare system, understand their care plans, and stay as independent as possible.
Companion Care & Social Support Services
Our companion care services are designed to reduce loneliness, encourage activity, and provide friendly support when family members cannot be present. Companion services are non-medical and may include:
- Friendly visits and conversation

- Games, reading, walks, and social activities
- Supervision for safety and reassurance
- Transportation to appointments or errands (if applicable)

Household Support & Light Assistance Services
Gracepoint Care Group also offers practical support around the home to make daily living easier and safer. These services are non-medical and may include:
- Light housekeeping and organizing
- Laundry and linen changes
- Meal preparation or meal planning assistance
- Trash removal and tidying
- Help with groceries and household tasks
- Assistance with heavy lifting or rearranging items for safety
This level of support is ideal for individuals who can care for themselves medically but need extra help maintaining their home and independence.
Appointment Assistance & Healthcare Advocacy
Navigating medical appointments can be overwhelming. Gracepoint Care Group offers RN-led appointment assistanceto ensure nothing is missed and your concerns are clearly communicated.
This service may include:
- Scheduling and coordinating medical appointments
- Transportation arrangement or accompanying the client (if desired)
- Preparing a list of questions and concerns beforehand
- Bringing updated medication lists and health information
- Taking notes during the visit and clarifying instructions
